1. In your Steam library rightclick on Sakura MMO 3.
2. Select Properties.

5. Once you're in location that the Sakura MMO 3 is installed in, go into folder game and paste the downloaded file, which will replace the original (censored) archive.

EXTRA STORY

I went first with Harem Route (because I believed it was most suited for Kotone/Viola given her personality) and after completing it ended up unlocking the Extra Story, which is accessible from Main Menu of the game under Extra tab. So you deinitely can unlock it with getting Harem Route. But I do believe you can unlock this part of the game after completing any of the positive endings.
